Continuity of Care for Persons with HIV and NCDs during COVID-19: Learning from the Chronic Care Model
PANCAP- PAHO Webinar: Continuity of Care for Persons with HIV and NCDs during COVID-19: Learning from the Chronic Care Model
In this PANCAP- PAHO Webinar presenters highlight recommendations and strategies to ensure access, initiation, uninterrupted treatment and continuity of care for persons with chronic conditions living with HIV, as well as persons with noncommunicable diseases (NCDs) during COVID-19.
There is an illustration of the Chronic Care Model as a person-centred approach that can be applied beyond NCD management to include People Living with HIV (PLHIV). This approach is been increasingly relevant as the prevalence of NCDs in PLHIV is expected to increase as populations age.
To discuss strategies to maintain essential services for people with chronic conditions and the importance of self-management.
